In-home advanced wound care

The wound doesn’t care that the drive is hard

With a physician’s order for ultrasound therapy, we bring advanced wound care and skilled physical therapy into the home — for patients who need it and are not currently under a home health agency’s plan of care.

The gap we fill

Between home health and no help at all

There is a large population of patients who are functionally homebound — a wound that cannot tolerate a shoe, a walker that will not fit a car, oxygen tubing, a caregiver who cannot lift — and who are nonetheless not currently receiving home health services. Perhaps an episode of care ended. Perhaps the agency’s therapists do not provide advanced wound modalities. Perhaps no one referred them.

Those patients frequently stop going anywhere. The wound is then managed by whoever changes the dressing, which is often a family member doing their best. Weeks pass.

We bring the clinic to them. The same evaluation, the same low-frequency ultrasound therapy, the same documentation and physician communication — delivered at the kitchen table.

The rules, stated plainly

What has to be true for a home visit

Home-based care under Medicare Part B has real requirements. We would rather you know them before the first call than after a denied claim.

A physician’s order

Ultrasound therapy requires a written physician order, and the plan of care must be certified by a physician. We handle the paperwork; your doctor signs it.

No overlapping home health

If you are currently under a home health agency’s plan of care, Medicare will not separately pay for the same services. Once that episode ends, we can begin.

Medical necessity for the setting

There must be a documented clinical reason the care is delivered in the home. Wound severity, mobility limitation, oxygen dependence, and transport barriers all count — they simply need recording.

Coverage decisions belong to Medicare, not to us. We document thoroughly and tell you honestly when something is unlikely to be covered, so you can choose to proceed as a self-pay patient with full knowledge of the cost.

What we bring through the door

  • Portable low-frequency ultrasound therapy equipment
  • Full wound assessment, measurement, staging, and photographic documentation
  • Dressing selection and caregiver training that outlasts the visit
  • Offloading, seating, and positioning assessment of the actual bed and chair you use
  • Gait, transfer, and strength training in the home you have to navigate
  • Fall-hazard review of the rooms you actually walk through

Service area

Home visits are offered across San Benito County and adjacent communities in Santa Cruz, Monterey, and Santa Clara counties — broadly the areas around our three offices — subject to scheduling and travel distance.

Not sure whether you’re within range? Call (831) 281-3328 and we’ll tell you in a minute.

Start with a conversation, not a form

Tell us about the wound or the fall, and we’ll tell you honestly whether we’re the right fit — and what your physician needs to send if we are.
