Wound evaluation
Full wound assessment, measurement, photography when clinically appropriate, vascular and nutritional screening, and an individualized plan of care.
Bring: physician orders if available, recent wound notes, current dressings, insurance cards, and a medication list.
Rehabilitation evaluation
Evaluation for orthopedic, balance, neurologic, vestibular, gait, strength, mobility, or chronic pain concerns.
Bring: relevant imaging or surgical reports, insurance cards, and a medication list.
In-home visit
Home visits are scheduled by phone after we confirm clinical appropriateness, payer requirements, home-health status, and travel distance.

Before your first visit
Complete your fillable intake packet at home
The packet can be filled out electronically in a compatible PDF viewer. When finished, save a copy and bring it with you, or print it and bring the paper copy.
